Once you schedule your first appointment, our team will provide new patient paperwork electronically.
Before your visit, please:
Fill out the new patient forms provided
Prepare a list of any current medications
Bring your dental insurance information
YOUR FIRST VISIT
During your first appointment, we’ll take the time to get to know your dental history and discuss any concerns you may have about your oral health. Your initial visit will consist of:
A complete dental exam
An oral cancer and periodontal (gum) exam
X-rays and photographs
The Mountain Brook Smiles team will use your initial exam, X-rays, history, and personal goals to create a customized treatment plan and address all of your dental needs. Exceptional dental care begins with open communication, and we promise to speak candidly with you about any concerns we diagnose that may require further treatment as well as any additional recommendations or alternative treatments. Working together, we can achieve the healthy, beautiful smile you deserve.
